Cannabis Reduces Dementia Agitation: LiBBY Study Results

What the LiBBY Study Investigated in Cannabis and Dementia

A team of US researchers carried out the first large‑scale, placebo‑controlled trial examining whether a combination of THC and CBD can ease agitation in people with advanced dementia. The results, unveiled at the International Alzheimer’s Conference in London in July 2026, have been described as an important signal for cannabis‑based medicine because agitation remains one of the toughest behavioural symptoms to treat in dementia care.

What the LiBBY Study Investigated in Cannabis and DementiaSignificantly Less Agitation After Two Weeks To quantify agitation, researchers employed the Cohen‑Mansfield Agitation Inventory (CMAI), a validated 29‑item scale. After just two weeks, the average CMAI score in the cannabis‑treated group was 6.27 points lower than that of the placebo group—a difference that remained statistically significant through the twelve‑week endpoint. Clinical impressions mirrored the scale data. Treating physicians rated improvement in 83.9 % of the cannabis arm after two weeks, compared with only 30.5 % in the placebo arm. By week twelve, the responder rates were 87.2 % versus 23.6 %. Study lead Dr. Jacobo Mintzer noted that such a response rate is unprecedented in dementia trials. Safety was a key focus. The incidence of adverse events was similar between groups (46.7 % with THC/CBD vs. 42.4 % with placebo). Most reported events were infections and gastrointestinal complaints, which are common in this frail, end‑of‑life population and were not clearly linked to the study medication. Why These Results Are Relevant for International PracticeAssessment and Open QuestionsFrequently Asked Questions

The trial, formally titled the Life’s end Benefits of cannaBidiol and tetrahYdrocannabinol (LiBBY) Study, recruited 120 participants diagnosed with Alzheimer’s disease or another form of dementia. All were in the late stage of the illness, qualified for hospice care, and suffered from severe agitation. Using a randomized, double‑blind, placebo‑controlled design—a methodological gold standard—the study assigned half of the patients to receive an oral preparation containing purified THC and CBD, while the other half received an identical‑looking placebo.

The dosing regimen consisted of 8 mg THC and 400 mg CBD per day, with the first week starting at half that amount to assess tolerability. Treatment continued for twelve weeks, and study visits were performed in participants’ homes or care facilities by investigators from ten medical centres. Funding came from multiple sources, including the US National Institutes of Health and the Alzheimer’s Association.

Why These Results Are Relevant for International Practice

Agitation and aggression are among the most distressing symptoms for people with advanced dementia, their families, and caregivers. Antipsychotic drugs are frequently prescribed despite clear evidence linking them to heightened stroke risk and increased mortality in dementia patients. A better‑tolerated alternative could therefore represent a meaningful advance in symptom management.

In jurisdictions where medical cannabis is legally accessible, clinicians can already prescribe THC‑ and CBD‑containing products on an individual basis, even if the exact formulation used in the LiBBY Study is not universally approved. Understanding how cannabinoids affect older adults—and what precautions are warranted—is essential; existing literature on cannabis use in seniors provides a useful foundation for these considerations.

The market is also responding to the aging demographic, with a growing number of hemp‑derived products marketed specifically to older consumers. Nonetheless, any therapeutic application should be guided by robust clinical evidence and individualized risk‑benefit discussions.

Assessment and Open Questions

While the findings are encouraging, several caveats temper enthusiasm. The LiBBY Study deliberately enrolled participants nearing the end of life; therefore, the results may not generalize to individuals with mild or moderate dementia. Furthermore, the data have so far been shared only as a conference presentation and have not yet undergone peer‑reviewed publication. Independent replication and longer‑term safety studies will be needed before the approach can be endorsed as a standard clinical recommendation.

Nevertheless, the trial adds to a growing body of research suggesting that cannabinoids can influence neurological pathways implicated in dementia. Earlier investigations of CBD alone in Alzheimer’s models have hinted at anti‑inflammatory and neuroprotective properties, and the LiBBY results build on that foundation by demonstrating a clinically meaningful behavioural effect in humans.

Frequently Asked Questions

Can cannabis reduce agitation in dementia?

The LiBBY Study provides the first strong evidence from a large, controlled investigation that a THC/CBD combination significantly lowers agitation in advanced dementia compared with placebo. Further research is required before broad treatment guidelines can be issued.

What doses of THC and CBD were used in the study?

Participants received an oral preparation delivering 8 mg THC and 400 mg CBD each day. During the initial week the dose was halved to assess tolerability, then increased to the full amount for the remaining eleven weeks.

Is this cannabis preparation widely available?

The specific formulation tested in the LiBBY trial is not yet approved in all regions. However, in countries where medical cannabis is permitted, physicians may prescribe THC‑ and CBD‑containing medicines on a case‑by‑case basis.

Was the treatment well‑tolerated?

Side‑event rates were comparable between the active and placebo groups (approximately 45 % vs. 42 %). The most frequent complaints were infections and gastrointestinal issues, which are typical in this severely ill cohort irrespective of study medication.

Who conducted the LiBBY Study?

Researchers from the Medical University of South Carolina and Georgetown University led the trial, working under the auspices of the NIH‑supported Alzheimer’s Clinical Trial Consortium. Findings were presented in July 2026 at the Alzheimer’s Association International Conference in London.
